Standards for Mathematical Practice Survey

Using the ranking scale below, please assess to what degree your students do the following during math instruction:
1 = 1-2 times per month
2 = 1-2 times per week
3 = 3-4 times per week
4 = 4-5 times per week
5 = within every lesson
